Health Assessments:
Health assessments are a Medicare initiative aimed at allowing people who may be at risk of illness or who are in a particular age bracket, to have a comprehensive health assessment completed. Health assessments are performed by the practice nurse and your GP. Health assessments are bulk billed so there is no out-of-pocket cost to you. A list of health assessments are below.
Healthy Kids Check:
A Healthy Kids Check is for every four-year-old child in Australia. The Healthy Kids Check will promote early detection of lifestyle risk factors, delayed development and illness, and introduce guidance for healthy lifestyles and early intervention strategies
45 to 49 Year old Health Checks
This health check is targeted at people between the age of 45 and 49 years of age (inclusive) who are at risk of developing a chronic disease. A chronic disease or condition includes the following but is not limited to, asthma, diabetes, mental health conditions, arthritis, musculoskeletal conditions, cancer, cardiovascular illness.
Over 75 Years of Age Health check
This is an annual health assessment for those in this age group.
Comprehensive medical assessment for residents in age care facilities
This is an annual assessment completed onsite at the aged care facility to asses the patients health, physical and physchological function.
Aboriginal & Torres Strait Islander
Health assessment for patients that have identified as ATSI.
Type 2 Diabetes Risk Evaluation
Type 2 diabetes risk assessment evaluation is for people between the age of 40 and 49 with a high risk of developing diabetes. Are you at risk? Please make and appointment to see your GP today.
